To create a chaos item you need:

1. Jewel of Chaos
2. Item +4, +4 option (add Jewel of Life)

To increase chance of succses, add more upgraded items.

A successful combination produces:

Chaos Dragon Axe,

Chaos Nature Bow,

Chaos Lightning Staff.
